My Pledge Regarding Your Health Information
I understand that health information about you and your care is sensitive and personal. I am committed to protecting it and holding it in the highest confidence. During our work together, I create and keep records of the care and services you receive from me. Unless specific authorization is provided in writing by you, I will not disclose your personal health information, treatment plans, payment or health care services to any medical provider, insurer, individual or organization.
The Ways I Use Your Information
I keep “client journey” notes. Any use or disclosure of these notes requires your authorization unless used to provide recommendations to you, for my use in defending myself in legal proceedings instituted by you or as required by law, including to help avert a serious threat to the health and safety of you or others.
I may use your personal information to contact you to remind you that you have an appointment with me.
I may use and disclose your information to tell you about treatment alternatives or other services or benefits that I offer.
The Ways I Will Not Use Your Information
I will not use or disclose your personal health information for marketing purposes.
I will not sell your personal health information.
Your Rights with Respect to Your Information
You have the right to ask me to contact you in a specific way (for example, by home or office phone) or to send mail to a different address. I will agree to all reasonable requests.
You have the right to see and get paper or electronic copies of your personal health information and other information that I have about you, other than “practitioner journey notes”. I will provide you with copies, or a summary of them, if you agree to receive a summary, within 30 days of receiving your written request.
